What is platelet rich fibrin matrix (PRFM)?

PRFM, or platelet-rich fibrin matrix, involves the use of platelets taken from your own blood. Platelets are normally involved in clotting but have other beneficial properties including making growth factors that stimulate healing of bone and soft tissue. Dermatologists have noticed beneficial effects of PRFM in strengthening and growing existing hair follicles, building collagen, and speeding healing after laser procedures. Although not FDA approved specifically for these indications, many dermatologists use this technique in aesthetic treatments with published studies showing benefit.

The PRFM system used by Modern Dermatology includes a step in which the clotting cascade is activated, stabilizing the platelets and allowing for a sustained release of important growth factors over the several days following injection. This is makes PRFM favorable to PRP (platelet-rich plasma) as the treatment area allows longer exposure to the growth factors to improve results. Multiple treatments are advised at monthly intervals to initiate cell growth, with subsequent maintenance treatments on an as-needed basis. Results are noticeable 3 months after the first treatment and continue to improve.

Am I a candidate for PRFM?

PRRM is appropriate for both men and women to help thicken aging skin and speed healing after aggressive lasering.

Is it safe?

Yes, PRFM is safe as it uses your own blood components.

What is my preparation for the treatment?

We suggest that you continue to take all your prescribed medications, up to and including the day of treatment. You should avoid aspirin and other non-steroidal anti-inflammatory medications (Aleve, Motrin, Ibuprofen, Advil) ten days prior to the procedure. These medications impair platelet function and promote bleeding. Unless otherwise directed, you may continue to use topical retinoids (i.e., tretinoin, Retin-A, Triluma, Melasma Quadruple Cream), antioxidants, and/or hydroquinone up to the day of your treatment.

What is the treatment like?

Blood is taken from your arm in the same way as a routine blood draw, then spun in a centrifuge to separate the blood components. Plasma, which contains the platelets, is separated, activated, and injected into the skin with multiple injection sites over the treatment area. 

What should I expect after my treatment?

Immediately following the procedure, swelling will occur but should resolve in 3-7 days. You may have temporary redness or bruising at the injection sites. Bruising can be minimized by avoiding blood thinners (ibuprofen and aspirin) 10 days before and not drinking alcohol the night before treatment.

Are there any risks or possible side effects?

  • Bleeding – can occur after blood draw or after injections. As no actual incisions (cuts) are made, the bleeding can be controlled with pressure.
  • Infection – you should not have the PRFM procedure performed if you have an active infection in the treatment area or in the area where the blood draw will be done. Rarely, a bacterial or viral infection can occur at the sites of PRFM injection. If this occurs, your provider may prescribe medications to treat the infection.
  • Damage to deeper structures – any injection can cause damage to nerves, blood vessels, or muscles. However, this would be exceedingly rare with PRFM given the relatively superficial nature of the injections.
  • Allergic reaction – if lidocaine is used for numbing, allergic reactions can occur to the lidocaine or the preservative. PRFM itself is autologous (from your own body), and so allergy is not a concern.
  • Insufficient response- we cannot guarantee your results from PRFM. Most patients are happy and there is very little risk. However, for unknown reasons some will have a sub-optimal result. Lifestyle, health, genetics, and normal aging can all play a role.

Are there any contraindications?

PRFM is contraindicated for people who are pregnant or breastfeeding, those with platelet or blood disorders, if there is active skin disease or rash in the treatment area, if there is an untreated malignancy in the treatment area, or if the individual has a localized or systemic infection.
